Our aim is to continually improve health and wellbeing for all in Sefton.
The development of our Sefton NHS Five Year Place Plan “Reshaping Sefton” looks to set-out our ambitions and priorities for the next five years.
Our plan has been deliberately designed to underpin the Sefton Health and Wellbeing Strategy, which is led by Sefton Council, to ensure that the health, care and wellbeing for the people of Sefton are understood and catered for.
We want to help you to help yourself be healthier and happier, but also ensure we have the right services and activities in place to support people, whatever their needs.
To do this we have studied the results of the recently refreshed Sefton Joint Strategic Needs Assessment (JSNA) - which helps tell all partners where the support is most needed, and the varying needs of our communities. Our plan outlines the NHS contribution to improving health, care and wellbeing with partners across Sefton.
A lot has been achieved over the last five years, since the development of our first Shaping Sefton Strategy. We have listened to what you told us in 2014 and either put in place or are in the process of developing better and more focused health, care and wellbeing to meet your needs.
The outline draft plan you see here is a development of that strategy and we want you to tell us if you feel we are heading in the right direction?
